If you have ever been told your BMI places you in the ‘overweight’ or ‘obese’ category — or conversely, been told your BMI is ‘healthy’ while you feel far from it — you’ll know that a single number doesn’t always reflect how you feel or how well you are. Here’s what BMI actually measures, why it matters clinically, and why good weight management practice goes well beyond it.
What Is BMI?
BMI, or Body Mass Index, is a simple calculation: your weight in kilograms divided by your height in metres squared. It was developed in the 19th century as a population-level statistical tool — not as a measure of individual health. Despite this, it became widely adopted in clinical settings because it is quick, inexpensive, and reasonably predictive of health risk at the population level.
A BMI of 18.5-24.9 is classified as ‘healthy weight’, 25-29.9 as ‘overweight’, and 30 and above as ‘obese’. These thresholds trigger different clinical considerations and, in some cases, determine access to NHS services.
The Limitations of BMI
BMI does not distinguish between fat and muscle. A person who is very muscular — including many athletes — may have a high BMI while having very low body fat and excellent cardiovascular health. Equally, someone with a ‘normal’ BMI may carry significant visceral fat (fat around the organs), which is metabolically far more dangerous than subcutaneous fat (fat under the skin).
BMI also doesn’t account for age, sex, ethnicity, or fat distribution. Research consistently shows that these factors significantly alter the metabolic risk associated with any given weight. People of South Asian or East Asian heritage, for example, tend to develop metabolic complications at lower BMI thresholds than those of European heritage.
What a Better Assessment Looks Like
A clinically useful assessment should include:
- Waist circumference — central adiposity is a more reliable predictor of metabolic risk than BMI. A waist above 88cm in women or 102cm in men signals significantly elevated risk.
- Waist-to-height ratio — increasingly used in research; a ratio above 0.5 suggests elevated health risk regardless of BMI.
- Blood markers — HbA1c, fasting glucose, insulin, lipid profile, and inflammatory markers give a much more complete picture of metabolic health.
- Body composition — where available, DEXA scan or bioelectrical impedance can distinguish fat mass from lean mass.
The Clinical Relevance of BMI
Despite its limitations, BMI remains a useful — if imperfect — starting point for clinical risk stratification, and it continues to inform prescribing thresholds for weight loss medication in the UK. Understanding where you sit on the scale is therefore useful, even if it shouldn’t be the whole story.
The key is to use it as one piece of a larger picture rather than as a definitive verdict on your health.
